New Patient Registrationregister_now

We are happy to accept patients irrespective of their colour, age, religion, medical history and sexual identity. To register please come in and ask a receptionist for a registration pack, fill it in and return it along with photo ID and proof of local address.  Alternatively you can visit https://www.nhsinform.scot/care-support-and-rights/nhs-services/doctors/registering-with-a-gp-practice where an online form can be completed.  Send it and the Adult New Patient Questionnaire (found below) to clinical.s76161@nhslothian.scot.nhs.uk. (Sending unencrypted emails is unsafe - You Do So At Your Own Risk.  As emails travel across the public internet, they can be intercepted, as such, there is no guarantee the email will be delivered to/received by the practice.  No email can be secured by NHS Lothian until it has been received.

In joining this practice you are free to consult any doctor of your choice within this practice.

We would like new patients to make an appointment with the nurse for a health check and any doctor for a new patient check so we can get some background and get to know you a little.

Medical treatment is available from the date of registration. Please contact reception for further information.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.


Alternatively you may print off a registration form and a new patient questionnaire, fill both in and bring them with you on your first visit to the practice.
