Sick / Fit Notes and Self Certification

When do I need a fit note (formerly a sick line) from the GP?

  • You don’t need to see a GP or nurse just to receive a fit note
  • If you are off work FOR LESS THAN A WEEK your employer will ask you to complete a self-certification form (SSP1 form)  which they will provide for you.
  • If you are off work for 7 days or more, you will need a fit note from the GP. We can only issue a fit note if we know you have been ill or because the hospital has informed us.
  • Any requests for a fit note for illness less than 7 days is unnecessary and would only be done for a standard fee.
  • If you need to extend a fit note you already have please note that this can only be done from the date your fit note runs out (not in advance!). We may ask you the reason for the extension.

What is an amended fit note?

In some circumstances your employer may allow a partial return to work for someone recovering from an illness or injury. This may be a ‘phased return’ or one with amended duties or hours of work. These recommendations can be added to the fit note. Please let us know if this is required.
